Does a Loveland driveway gate need fire department approval?

It can, depending on where the gate sits. Loveland Fire Rescue Authority enforces the 2024 International Fire Code,¹ and its amendments carry an exception to the D103.5 gate rules for private driveways serving one single-family residence, as approved by the fire code official.² That exception is the fire code official’s decision, not an automatic pass. How tall can a front-yard fence be in Loveland?

Lower in front than elsewhere. Outside city limits, county rules come in.
